  • Is Cardiovascular Training Worth the Investment? Cardiorespiratory Fitness (CRF) Explained

    In this upcoming series of articles, we will address the pressing question, "Is Cardiovascular Training Worth the Investment?" To answer this question, we must first define what cardiovascular fitness is. Understanding Cardiovascular Fitness: Cardiorespiratory Fitness (CRF) Cardiorespiratory fitness (CRF) refers to the ability of the circulatory and respiratory systems to supply oxygen to skeletal muscles during sustained physical activity. It's a measure of how well the heart, lungs, and blood vessels work together to transport oxygen to the muscles and how efficiently the muscles utilize that oxygen to produce energy for physical movement. When we talk about cardiovascular training, we mean the process of improving cardiorespiratory fitness through specific exercises and adaptations made by the body. In essence, the goal of cardiovascular training is to increase CRF, enhancing overall health and lifespan. Why Is CRF Important When Evaluating the Return on Investment of Cardiovascular Training? Predictive of All-Cause Mortality: The relationship between cardiorespiratory fitness (CRF) and all-cause mortality is both profound and clear. A comprehensive study involving 122,007 patients (average age 53.4 years) observed over 1.1 million person-years revealed that all-cause mortality was inversely proportional to CRF. The most fit individuals, classified as elite performers, had the lowest risk of death, a significant 23% lower than even those considered high performers (1. Grundy et al., 2012). A critical aspect of this relationship is the association between a lower resting heart rate (RHR) and a lower risk of all-cause and cardiovascular disease (CVD) mortality. A normal resting heart rate for adults ranges from 60 to 100 beats per minute (bpm). A lower heart rate at rest can indicate more efficient heart function and better cardiovascular fitness. For example, a well-trained athlete might have a normal resting heart rate closer to 40 bpm. According to the medical journal Open Heart, men in their 50s with a resting heart rate of 75 bpm or higher are twice as likely to die of heart disease within 11 years than peers with a resting rate of 55 bpm or fewer. The relative risk of all-cause mortality was 1.12 for those with a resting heart rate of 60–80 bpm and 1.45 for those with a resting heart rate greater than 80 bpm (2. Saxena et al., 2013). The effects of endurance training on resting heart rate were further explored in the HERITAGE Family Study. This 20-week endurance training program found that subjects exhibited a 16.0% increase in VO2max and a significant decrease in resting heart rate. Despite the significant increase in VO2max, the decrease in resting heart rate was minimal, emphasizing the complexity of the relationship between exercise training and heart rate (3. Wilmore et al., 1996). In a comprehensive study conducted by (3.) Grundy et al. (2012), the relationship between cardiorespiratory fitness (CRF) and mortality risk was thoroughly examined. The findings provide a compelling illustration of this relationship, revealing that individuals with the lowest CRF faced a mortality risk approximately five times higher than those classified as elite performers. This significant disparity was not limited to a specific age group but was consistent across various ages, including the elderly. Such results emphasize the universal importance of CRF, regardless of age, and clearly highlight the critical role it plays in determining health outcomes. The tangible benefits of being fit, as contrasted with the potential liabilities of being unfit, are underscored by this study, reinforcing the essential value of cardiorespiratory fitness in promoting longevity and overall well-being. Predictive of Chronic Disease Risk: The correlation between cardiorespiratory fitness (CRF) levels and the risk of chronic diseases is well-established and significant. High CRF levels are associated with a decreased risk of prevalent chronic conditions such as heart disease, cancer, and diabetes (1. Grundy et al., 2012). This association is not merely theoretical; regular cardiovascular training that enhances CRF can serve as a formidable defense against these life-threatening conditions. Conversely, the data reveals a concerning trend: low CRF levels correspond with an increased risk of these very diseases. The gravity of this relationship is further emphasized by the World Health Organization (WHO), which recognizes cardiovascular diseases as the leading cause of death globally. In 2019, these diseases accounted for an alarming 32% of all global deaths. The WHO's stance is clear and backed by evidence: most cardiovascular diseases can be prevented. The key lies in addressing behavioral risk factors, including one that is both modifiable and crucial—physical inactivity (4. World Health Organization, 2021).
